SETTLEMENT NOTICE:
Council is working to prevent the IRS taxation of the Sept. 12 settlement payments, but as a precaution, an IRS form, a W-9, will be sent out with the August per capita payments. Everyone who wants to receive their Sept. 12 settlement payment must submit a W-9 form to the CSKT Enrollment office. Failure to submit a W-9 could result in a delay of sending out the Sept. 12 payment.
For tribal members who receive direct deposit, and do not receive a paper check, the W-9 form can be found by searching the IRS web site or by going to this web address, http://www.irs.gov/pub/irs-pdf/fw9.pdf or by clicking here.
For parents of children who be receiving their settlement, fill out the for with the child's information, then sign for your child.
Q. What happens if I do not submit a W-9 form to Enrollment?
A. The check will not be issued. If the payment is declared as taxable income (matter still pending), and the check is issued without a W-9 form in hand, both the individual and CSKT could face fines for each incident.
W-9s should be mailed to:
